2. The Care Navigator Service works closely with NHS Teams, GPs, Social Care, and Community and Voluntary Sectors groups and organisations to ensure that you are connected with the right services in your neighbourhood to meet your health and wellbeing needs. 3. Be Well Social Prescribing Service can support you if you’re experiencing social isolation; mental health difficulties or a long-term condition, or if you need help with lifestyle change, welfare rights, and employment advice (including furlough information and guidance). Support is available via telephone or video call during the covid-19 pandemic. Referral is via ANY organisation, including your GP practice.
